For classified participants

You have been selected to take part in the program Erasmus + Traineeship. This means you will get a grant for up to three months of traineeship.Check how to prepare finaly.

EMLPOYER SEARCHING
All students that are qualified but haven’t found the employer yet, will be required to declare the choice of the employer in appointed term.


DOCUMENTS WORKFLOW


1. Before the mobility
Participant has to download the document from the website: http://www.kariery.uek.krakow.pl/zrealizowane-projekty-1/erasmus-plus/dokumenty-do-pobrania;
Participant has to fill the gray part (.doc form);
Participant has to send it to the employer (word format);
Employer has to fill the blue part, sign it and return the scan of the document to the participant;
Participant has to print a document, sign it and deliver to the ACK;
Document has to be signed by the ACK employee;


2. During the mobility
Participant has to send a required mobility questionnaire one month after the beginning of the mobility (to download);
In case of any changes about the mobility, the participant has to send by e-mail completed and signed by the employer part DURING THE MOBILITY, minimum a month before the end of the mobility, to accept the changes by ACK.

3. After the mobility
Participant receives from the employer the original documents: all three parts of the LEARNING AGREEMENT. In case there were no changes during the mobility, only two required parts: BEFORE THE MOBILITY and AFTER THE MOBILITY.
On the BEFORE THE MOBILITY part there are dates from before the mobility, on the part AFTER THE MOBILITY - the date after the end of the mobility or from the last day of the mobility.
Participant has to deliver the document to the ACK office on time. Ask your employer for the documents in 2 copies (especially the part: AFTER THE MOBILITY) because we have to keep the originals, and a certificate of mobility will be useful for you in the future.

LANGUAGE TESTS
Before the beginning of the mobility and at the end of the mobility each participant of the Erasmus + program is required to complete the proficiency test of the language of the mobility abroad. The access to the on-line test will be sent to your e-mail before and after the mobility.
This test has to determine the level of your language proficiency. It has no influence to the participation in Erasmus +.
Please note that the obligation of completing the test is a part of the contract. You can be asked for reimbursement of the grant if you will not complete the test.
Students who complete the test with the lowest score will receive a license for a mandatory on-line course in order to improve their knowledge.


SIGNING A CONTRACT
It’s required to sign the contract between the University and the student up to 21 days before the start of the mobility in order to receive the grant.

Please note that before the meeting for signing the agreement, you are required to:
1. Inform us beforehand, so we will be able to prepare a language test for you;
2. send us an e-mail with attached and completed file a statement of bank account number in the .doc form.

To sign a contract it is required to provide to the ACK office the following documents:

Signed statements with a bank account number;

  • Learning Agreement;
  • The consent of the Dean's office at the IEP * (in case of a student’s mobility during the academic year). Consent is granted on the basis of a certificate issued by ACK.
  • If you plan to go for the traineeship after the finals and the end of the course, you need a Dean’s approval.
  • Certificate from the Dean's office of completing the final semester prior to departure;
  • Consent promoter for the mobility - if you are studying in the final academic year and you take part in mobility with a student’s status;
  • Certificate of graduation – in case of the post-graduate internship;
  • Copy EKUZ card - the card from the NHF;
  • Copies of the insurance policy;
  • Language certificate from the The Online Linguistic Support (OLS);
  • Possibility to check the original documents by ACK;

Any person participating in the traineeship should take care of the arrangement of all matters related to the organization of studies in order to avoid surprises when you return. We encourage you to contact your Dean's office to obtain information on the regulations of student affairs before departure, and the possible continuation of studies after return.

* Each student going for the traineeship during the academic year should get a permission for the individual education program (IEP) before the mobility starts. IEP is the implementation of the current plan of studies. It allows students to make some adjustments for their schedule. It can concern either participating in certain classes or getting credits after completing an exam from the certain subject on the other day

SETTLMENT OF THE MOBILITY
Please settle your mobility up to the month of the finishing. In case of the mobility ended in September 2016 – till the 10.10.2017 r.

During that period it is required:
1. to provide to the ACK office the original document of Learning Agreement (all required parts) in a paper form (a printed copy is not accepted);
2. to provide to the ACK office the certificate of a student status from the Dean's office (students);
3. to complete the on-line Participant Report Form on the specified period;
4. to complete the language test The Online Linguistic Support (OLS) on a prescribed period;

You receive an e-mail with a Participant Report Form and The Online Linguistic Support automatically.
Students who already have found the employer are invited to the ACK office in order to receive the certificates needed for the National Health Fund and the Dean's office.
